We look at the ADC and this is what we see:

The ADC signal is low and so we can be confident that this is true restricted diffusion. The differential is now:

  • infarct
  • hypercellular tumour

There is associated mass effect, which can be seen in both oedema in an infarct and in a tumour mass. However, there is very strong, homogeneous restricted diffusion and the signal abnormality is in a vascular territory. These point towards it being an infarct. Also, if the onset of symptoms was sudden then an ifnarct is more likely.
